You're mixing different features that aren't really related.
First, work on getting a graph (not a template) working.  This
will require you to define a "DS template" as you've already
been doing.  For that, pick a hostname and try to build the
graph you want to see for that particular host.  From the
snapshots you sent, i think you'll have no trouble doing so.

Once that is done, and only then, you need to convert the
graph into a template.
